Sudden Loud Tone via USB

My CQ-18T has started to emit a loud tone (think fire drill levels) via the USB channel. This is random and has only happened twice before while watching YouTube. The only way to silence it is to mute the USB channel or unplug the USB B cable from the unit, then replug.

Base setup: Mac Studio (via USB) → CQ-18T → Adam Audio A7X

Thoughts?

Any chance there is a feedback loop between the mixer and computer that becomes un muted?
