It happened before - but yesterday i had to switch off Mixrack and turn it on again.
Had no time to switch off Monitors as i were stressed…
And as the Mixrack booted up and build the settings - the Monitors had a big feedback.
Maybe there could be a option to mute the outputs for 5 seconds or so till all settings are online when the Mixrack boots up.

I pointed this out a long time ago.
In my opinion, the problem lies in the fact that the preamps get a very high sensitivity for a moment when the phantom power is switched on again. it’s very short, so the feedback is very short. but it can be very annoying.
the better method has to be: mute the INPUTS for a moment; -)
it would therefore be very helpful if the time to unlock would be a little longer, so that the circuits can adjust themselves accordingly.
a mute until the mixrack is fully started will be nice, now there are 3 loud kracks when the system start after a failure. A mute inputs is active on all mics this is not a sollution.
