I recorded a 1khz sine signal.
I tried this after hard reset. In order to exclude wrong setting.

When you say ‘phone out’ is that the headphone output?
If so, then as you have completed a hard reset, this could indicate a hardware problem with the headphone amplifier.
If you are referring to the 1/4" Jack outputs on the rear however (occasionally referred to as phone sockets), then please fist check your cables and ensure they are both balanced and working correctly, as using an unbalanced cable for the right signal could also cause the difference in level displayed.
In the event that you do believe there is a hardware problem, please contact your local distributor (https://www.allen-heath.com/where-to-buy/) who will be able to test your Qu completely and carry out any repairs as necessary to return it to full working order.

Yes, phone out is headphone out.
And my TRS( 1/4″ Stereo Jack ) to two TS( 1/4″ Mono Jack) cable works correctly.
As you see, I recorded another audio signal from kamasan field mixer head phone out with the same cable.
I had already visited distributor today. But they said that can"t fix it.
I have to contact distributor again.

are you sure nowhere has the pan knob off centre to skew your signal

Check this and then L/R means stereo.

Is the problem with the sigma phones out, or the Qu phones out?
The original picture indicates it was the Qu?
So how does the sigma fit in this question?
Could you tell us ALL the hardware involved and all the connections between them,
as well as any software involved. I am having trouble seeing how your sigma fits with the Qu and how that related to your original question.
Where did you record the 1Kcps sine wave?
How are you playing it back on the Qu?
